NDIS Care Service Provider
As a registered NDIS service provider in Victoria, we deliver various support categories available under NDIS.
Optimum Care Group is an NDIS (National Disability Insurance Scheme) registered Service Provider since the early rollout out of NDIS (2018) in metropolitan Melbourne. We at Optimum Care Group are the specialists in providing support services to participants with Mental Health Disabilities, Physical Disabilities, Autism Spectrum Disorder (ASD), Intellectual Disability, Deafness, Attention Deficit Hyperactivity Disorder (ADHD), Anxiety Issues and many other disabilities. Our support workers and staff members are professional, friendly, compassionate and are qualified and experienced holding all the relevant checks and insurances.
We have a range of experienced male and female disability support workers, different ages groups (young,mature) and support workers that can speak different languages, support workers that can sign (Auslan) and support workers with psychology background. Optimum Care Group values our participants and offers them a complementary one hour meet and greet with our support workers to allow the participant try our services.
Our experienced and caring team provides supports to participants who require help with household tasks, domestic cleaning, personal care, social and community participation, support coordination services and podiatry services. We provides support to participants who are self-managed, plan managed, NDIA and privately managed.

Connection: Assist the Participant to develop knowledge, experience and connections with the community and broader systems of support.
Support Design: Works together with participant to understand Plan funding and its purpose. Support Coordinator will understand the Participant’s confidence and skills, and helps participant identify what they want from services. Will develop and design support solutions to meet Participant outcomes.
Establish Supports: Assist the Participant to identify and consider support options, and link the Participant to the broader systems of supports. Where practical creates a supports and action plan to facilitate the participant to implement their plan.
Crisis: Assistance to resolve points of crisis and developing capacity and resilience in the Participant's network.
Coach, Refine, Reflect: Coach the Participant through challenges that come up. Helps Participant prepare for review and report on achieved Participant outcomes.
